Registered Field Nurse

Job Description:

UJC is seeking the right individual to join our team of nurses.

Qualifications:

  • Current license to practice as a Registered Nurse in New York State (BSN)
  • Experience as an RN in a home care setting (preferable)

Duties:

  • NO UAS VISITS
  • Initial client visit, supervisory and reassessment every 6 months
  • Completing 485’s
  • Planning and implementing plan of care
  • Evaluation visits for HHAs and PCAs
  • Conducting HHA training classes and competency
  • Post-Hospital visits & emergency visits
  • Conducting in-service classes if needed

Language: Bilingual - Spanish/English

Job Type: Per Diem/Part-Time

Salary: $40-$50 per hour depending on experience

 

Masaryk Towers NORC Nurse

Overview:

The NORC Nurse works as a member of the interdisciplinary care team responsible for the integration of health-focused information, education, assessment, advocacy, and linkage to care for residents who live within the boundaries of Masaryk Towers. The goal of all interventions is to engage residents in self-care management and health strategies that support successful community living.

Education Requirement:

Registered Nurse license

Duties and Responsibilities:

Provides regularly scheduled blood pressure screening sessions for NORC residents. A brief health assessment is conducted during these sessions to all participants and blood pressure monitoring is provided over time.

  • Performs initial and periodic healthcare assessments of NORC members identified by the Case Manager to be at-risk. Determines their need for healthcare assistance or healthcare management
  • Provides health education, medication review, provider referrals, linkage, advocacy, monitoring of blood pressure and chronic illness, as well as healthcare coordination to the extent needed, helping individuals manage their chronic conditions, respond to acute episodes, and access healthcare.
  • Performs home visits, including safety assessments to prevent falls, in collaboration with Case Manager
  • For clients in need of Healthcare Management, develops an assessment and care plan, arranges and coordinates services on the resident’s behalf. Follows up with client at least once a month
  • Completes Health and Wellness assessments, Nursing Checklist, Case Notes and Worker Log for units of service documentation, as well as any other recordkeeping and reports in a timely fashion
  • Helps clients access the healthcare system by assisting with scheduling appointments, coordination with pharmacies and physicians, and linkage to home care agencies and other providers, as needed
  • Documents all nursing interventions provided as required by site location and enters information into the site database promptly
  • Performs other related duties as needed

Type: Part-time (2 days per week)

Salary: $36,000 per year
