Registered Field Nurse

Job Description:

UJC is seeking the right individual to join our team of nurses.

Qualifications:

  • Current license to practice as a Registered Nurse in New York State (BSN)
  • Experience as an RN in a home care setting (preferable)

Duties:

  • NO UAS VISITS
  • Initial client visit, supervisory and reassessment every 6 months
  • Completing 485’s
  • Planning and implementing plan of care
  • Evaluation visits for HHAs and PCAs
  • Conducting HHA training classes and competency
  • Post-Hospital visits & emergency visits
  • Conducting in-service classes if needed

Language: Bilingual - Spanish/English

Job Type: Per Diem/Part-Time

Salary: $40-$50 per hour depending on experience

 

Case Manager NORC

Overview:

The Case Manager is instrumental in furthering the NORC program’s mission to enhance the quality of life and independence for older adults as they age in place in their community. The Case Manager works under the joint supervision of the Program Director.

Education Requirement:

  • Minimum BA/BSW required.
  • Supervised casework experience with seniors preferred.

Duties and Responsibilities:

  • Engage older adults in their homes, in the NORC office, and in other community settings to gather information about their strengths, needs, abilities, and preferences.
  • Assess older adults’ presenting problems using a multidimensional psychosocial assessment, the health indicators survey tool, and the Patient Health Questionnaire.
  • Partner with older adults to establish a plan of care to address presenting problems.
  • Implement interventions indicated by the plan of care, including benefits and entitlements, advocacy, referral to community resources, problem-solving, and supportive contact.
  • Maintain regular contact with older adults to ensure timely follow-up to presenting problems.
  • Update older adults’ contact information, consents, and assessment data on an annual basis.
  • Evaluate older adults’ plan of care to ensure that the services provided are effective.
  • Participate in case conference and bi-weekly team meetings with NORC program staff.
  • Provide regular updates to the Program Director about older adults’ presenting problems.
  • Case work counseling and respond to crisis situations as appropriate.
  • Promote the weekly NORC health promotion activities and/or events via Outreach, flyers and networking other group programming.
  • Answer incoming telephone calls and greet older adults as they come into the NORC office.
  • Document all services provided to and on behalf of clients in the electronic database via STARS-Peerplace via detailed case notes and submit monthly worker logs.
  • Maintain confidentiality of all client information and adhere to the New York City Department for the Aging policies and professional standards of conduct.
  • All other duties as needed.
 

Part-time Custodian NORC

Overview:

Performs custodial maintenance duties, including dusting, mopping, finishing/buffing floors, setting up for classes, cleaning and restocking restrooms.

Duties and Responsibilities:

  • Performs general, routine custodial duties, including dusting, mopping cleaning restrooms, and restocking paper and soap supplies.
  • Performs routine maintenance of custodial equipment and supplies.
  • Empties trash receptacles, disposes trash into compactors and/or dumpsters, and bags trash for proper disposal.
  • Unlocks doors and ensures doors are locked after cleaning areas.
  • Reports needs and concerns to appropriate staff, which may include submitting or recommending work orders and checking supply levels; for positions within the residential buildings, responsible for reporting concerns related to community residents.
  • Ensures proper care in the use and maintenance of equipment and supplies; promotes continuous improvement of workplace safety and environmental practices.
  • Sets up and breaks down room for various programs.
  • Performs cleaning functions specific to the assigned facilities and/or based on seasonal/project requirements.
  • May provide vendors with access to areas for cleaning and may answer basic questions related to the service needed. May report issues with vendors to supervisor.
  • May assist or perform snow removal activities and minor maintenance, such as replacing light bulbs, adjusting furniture, or other similar activities.
  • Overall maintenance of the NORC front entrance and surrounding facility.
  • Performs miscellaneous job-related duties as assigned.
