Lillian Wald OAC Assistant Director

Role

Under the general supervision of the Director, assist in the administrative responsibilities of programming, community outreach, collaboration with other agencies and events/activities. This position requires cross functionality.

Requirements

  • Bachelor’s degree or a minimum of five years of professional experience working with older adults in a congregate setting.
  • Bilingual: English/Spanish

Responsibilities

  • Recruit, supervise and coordinate activities.
  • Manage and oversee scheduling of programming and activities.
  • Designate room set-up for activities in collaboration with all relevant staff/consultants.
  • Create Monthly Activities Calendar.
  • Manage scheduling of and communication with consultants.
  • Work with Director in program planning and development.
  • Assist in inventory control accountability of supplies.
  • Ensure equipment is in running order.
  • Maintain close attention to the welfare of the physical space.
  • Compile statistics, prepare reports, keep attendance and other required records for all programs.
  • Data Management: Entering units into database (Peerplace, STARS, VIVE, etc…)
  • Manage incoming communications, telephonic and electric.
  • Assist Director with compliance of auditable standards and requirements.
  • Represent agency in community and interagency activities.
  • Plan and execute events and special activities.
  • Promote the center through ongoing outreach endeavors.
  • Preform any other duties as recognized by the Director.

Case Manager - Supportive SRO Housing

Role

Under the supervision of the Program Director, the Case Manager will work to provide a range of support and services to the residents of a Supportive SRO Housing Program (Single Room Occupancy). The goal is to encourage and enhance wellness and self-sufficiency.

The Case Manager will promote independent living by providing structured case management services. The scope of services will assist each resident to live permanently, safely and independently.

The Case Manager will also be responsible for weekly contact with the client, monthly case notes, room checks, floor and community meetings, as well as escorting residents for entitlements, medical and psychiatric services when needed.

Requirements

  • B.A.
  • At least 2 years of case management or related experience working with the homeless, mentally ill and individuals with co-occurring disorders.
  • Be familiar with Microsoft Word and email.

Responsibilities

  • Direct service to caseload of 22 residents.
  • Maintain a caseload of 22 residents.
  • Develop a one-on-one therapeutic relationship.
  • Assist the residents to establish and maintaining a sobriety centered positive social lifestyle.
  • Complete intake/monthly summaries, service plans and progress notes.
  • Assist residents in obtaining and maintaining appropriate medical, psychiatric, and substance abuse treatment.
  • Assist residents in obtaining and maintaining entitlements, e.g. SSI, Public Assistance, Medicaid, housing and employment.
  • Assist residents in establishing and/or maintaining family and social contacts.
  • Assist residents with money and medication management needs
  • Provide crisis intervention services as needed and maintain case records to the standards required by governing agencies.
